COVID Patient Monitoring and Contact Tracing

Description 
App: Developing a system where patients who have tested positive for COVID-19 can use a smartphone based application to submit their oral temperature as well as other symptoms.  
Dashboard: Healthcare providers have a dashboard to view these results to monitor their condition.  
Contact Tracing: We will collect contact information (name/phone/email) from COVID-19 positives of whom they have been in close contact with to follow up with them to determine if they should be screened for COVID-19.
