Moxe Health - Bi-directional EMR Integration - Convergence

Description
Convergence replaces the paper and portals that payers traditionally use to send data to clinicians. Convergence is a bi-directional application for sharing insights that is embedded directly within the EMR. As insurers identify information needed for HEDIS or risk adjustment, they can send this data—and all relevant context—through Convergence,so that clinicians can review and act on the data without interrupting their workflow. The application uses single-sign-on to minimize effort needed from clinicians.

This implementation of Convergence focuses on HCC capture for Medicare Advantage risk adjustment. As our health plan partner identifies undocumented conditions, these insights and supporting evidence are shown to clinicians who can provide the update needed to close the gap in documentation.

As of June 2019, the Convergence application has now been live for almost a year. Clinicians have used the application to close hundreds of gaps in documentation, all without paper or portals!
