Integrate Home Health Care Data to ER and Urgent Care Facilities to reduce Hospital admission

Description
By using a combination of ADT messaging and C-CDA documentation standards, this pilot program between HealthCare Synergy and Great Lakes Health Connect goes to show that providing the information from a post acute provider to an Emergency Room or Urgent Care Facility upon admission will reduce re-hospitilization rate, by providing the ER or Urgent Care Facility with current patient data that would otherwise not be accessible.
