Transitions of Care Service - MiHIN

Description
The Transitions of Care service gives healthcare providers and care teams early warnings for critical patient health events.  The service provides clinical alerts, including medication reconciliation messages, to any provider who has declared an Active Care Relationship with a patient when that patient is admitted, discharged or transferred by a participating hospital or acute care facility, allowing care teams to proactively address patient care following discharge. More than 97% of admissions statewide in Michigan are currently reported through this service.
