• Please provide the link to the attendee page for the event 

Event and Presentation Details

• Name of the person or persons invited to the event
• Name of the event 
• Date of the event 
• Start and End Time of Event (in Eastern Standard Time)
• Format of Event: (in-person or virtual)
• Description of the event 
• Theme or Purpose of the event 
• Proposed topic and questions (Please provide proposed questions or topics you would like discussed. Please do not leave this field blank. It is okay if the questions are not final, but we need the questions to move forward with the approval process.)
