LTPAC Project - UHIN

Description 
As part of our Interoperability for Healthier Communities grant from the ONC, this project focuses on improving the transition of care when a hospital discharges a patient into the care of an LTPAC facility. In this pilot project, a hospital system is implementing an automatic discharge summary triggered by the discharge planning. This summary will be sent directly to the LTPAC facility via IHE protocols and the cHIE (Utah's HIE).
